First Visit

On your first visit, we want you to be as comfortable as possible. There will be a few forms that will need to be filled out to start the process. These include a privacy notice and consent form and a personal information form. You can go here to download and print off these forms.

Filling them out prior to your first visit will save you some time in the office. After the necessary paperwork is taken care of, the doctor will ask you questions to find out more about your current condition. This will help the doctor to know how to best help you and what areas to examine.

Next, a thorough physical examination will be performed to assess various systems of the body such as the nervous and musculoskeletal systems. This most often can be done without you having to undress.

The exam may include among other things: taking temperature, blood pressure, going through movements to assess motion and feedback about any pain associated with the motions. If it is determined through the examinations and questions that x-rays are needed, they will be taken at this time also.

Often the doctor will begin treatment on the first visit after a brief explanation of the findings from the exam. This is to help begin the process to recovery as soon as possible.
